Matching Donations:  
Please note that many companies offer
employee matching donation programs.  
Therefore, please check with your Human
Resources department and verify if your
company will participate.  Last year an
additional $1,300 was raised from matching
donations and we have lots of room for
improvement.  The LPBC is happy to help
fill out any necessary paperwork that is
required for the matching donation.

Make a Donation:  
Donating is easy.  Just click the button

complete your donation.  100% of every
donation goes directly to the LPBC

WAIVER & RELEASE FORM

PRE-REGISTRATION is currently closed, but everyone looking to participate can still
register on Saturday at the beach.  
All Polar Bears are REQUIRED to read and sign
that you agree to the terms and conditions of the
Waiver & Release Form.  If you
sign up at the beach you must physically sign the
Waiver & Release Form.  Click on

Registration Costs

- $20/swimmer when registering in advance via our online system
 - EARLY REGISTRATION CLOSES AT 5PM on THURSDAY, JAN. 26th.

- $30/swimmer the day of the event

website to sign up.  Registration fees go towards helping pay the permit fees imposed
by the City of Chicago.  Any extras will be given to the 2012 Families.
